Child Death Review underway into Octavia Swarts case

While a teen accused remains behind bars in connection with the death of 10-year-old Octavia Swarts in Kraaifontein, authorities are also trying to determine how to prevent another similar tragedy. 

 

The Department of Social Development confirmed that a Child Death Review is being conducted, which seeks to establish any child-protection risks, lapses in support, and identify opportunities for earlier intervention.

 

The department stressed that the Child Death Review is separate from the criminal investigation into Octavia’s death. The review will be conducted by a Child Death Review panel led by the Department of Health and Wellness’s Forensic Pathology Services, with representatives from Social Development, the National Prosecuting Authority and the South African Police Service. 

 

Octavia was found dead in a dam in Joostenbergvlakte last week.  

 

Social Development MEC, Wendy Kaizer-Philander, said it’s now their responsibility to learn from this tragedy and strengthen “the protective systems” for every child in the province. 

 

“The death of 10-year-old Octavia Swarts is deeply heartbreaking. No family should have to experience the pain of losing a child in these circumstances. It reminds us once again of how vulnerable our children can be and of the responsibility we carry, as government and as a society, to protect them,” said Kaizer-Philander. 

 

The 14-year-old accused of her death, who is said to have been a relative of the victim, is currently awaiting his next appearance on Monday for a bail application.

Meanwhile, Kaizer-Philander noted that support has been made available to Octavia’s family. She said her department would monitor developments closely and ensure any further actions required by the panel are followed. 

 

Kaizer-Philander said she attended the Grade 4 learner’s memorial service, hosted by Joostenberg Primary School on Monday. Family, friends, classmates and members of the community gathered to remember her. The school said Octavia would remain part of its school family and that her place would never be forgotten.
