Four warders at the Goodwood Correctional Services Facility have been suspended, following the death of an inmate, Quinton Fortuin, in February.

According to the department’s official statement, the incident occurred on the morning of 16 February 2025 during the daily unlock procedure, where inmates are released from their cells and counted. While other inmates exited promptly, Fortuin reportedly remained in his cell and only joined the line when instructed.

At approximately 08:10, Fortuin allegedly attacked correctional officer Mr Lwandile Manise with a sharpened object, causing serious injuries, including a broken arm. Manise was immediately treated and taken to hospital.

In response, additional officers intervened to subdue Fortuin. During the restraint, Fortuin sustained significant injuries and was transported to Karl Bremer Hospital, where he was pronounced dead shortly after midday.

National Correctional Services Commissioner, Makgothi Thobakgale, said a post-mortem confirmed Fortuin’s death resulted from multiple blunt force injuries. The incident is under investigation by the South African Police Service.

“After reviewing the investigation report and considering its findings, I have decided that the Western Cape Region will reimpose the suspension of two officials, Mr. Lamla Shumane and Mr. Xolani Matshoba, who were previously suspended but later reassigned. They will now join Mr. Qhama Kweleta and Ms. Thembisa Mninzi, who remain on suspension. Other officials confirmed to have been involved will also be considered for possible disciplinary action,” said Thobakgale.
